The Prevalence and Associated Clinical Factors of Dizziness in Migraine and Tension-type Headache - Preliminary Study

Abstract
Background: Subjective complaints of dizziness are common accompaniments of primary headache disorder. We aimed
to assess the prevalence and associated clinical factors in patients with migraine and tension-type headache (TTH)
according to the presence of dizziness during headache period and headache-free period.

Methods: The consecutive patients with migraine or TTH who visited to university hospitals from January to April
2014 were selected. We assessed patient¡¯s perception about dizziness and compared the clinical parameters of headache, psychological status by Hospital Anxiety and Depression Scores (HADS), status of physical activity by Korean version of International Physical Activity Questionnaire (K-IPAQ), self perception of handicap by Korean Dizziness Handicap Inventory (KDHI) sores and severity of motion sickness according to the presence of dizziness.

Results: A total of 90 patients were enrolled: mean age of 39 years old; women 74%: 68 migraine, and 22 TTH patients. Dizziness during headache period was common in both migraine (36.8%) and TTH (31.8%). During headache-free period, nonspecific dizziness is more common in patients with migraine compared with TTH (25 vs 4.5%. p=0.03). The patients with dizziness during headache period have more severe VAS (p=0.02) and higher KDHI scores(p<0.001). The patients with dizziness during headache-free period have more monthly headache days (p=0.02), less days of muscle strengthening activity (p=0.04), higher depression (p=0.007), and anxiety scores (p=0.03).

Conclusion: Dizziness during headache period is associated with more severe headache, whereas dizziness during headache-free period is related to multifactorial condition. So, subjective dizziness associated with headache may be part
of more severe headache manifestation.
